Medication

Over-the-counter and prescription medications fall under the category of abortive methods. These methods are reactionary in nature and often relieve pain quickly or immediately, but do not lessen the severity or frequency of the headaches, nor do they prevent the recurrence of the headaches.

Injections

Interventional Pain Management techniques can be highly effective in treating headaches. These techniques may include intramuscular narcotic injections such as Demerol or Toradol to control pain. Nerve blocks, injections which paralyze nerves that transmit pain, can also be used by the physician to control pain. A few drugs used in nerve blocks include Botox, Depromedrone, and Sarapin.

Surgery

Towards the end of the Interventional Pain Management spectrum is the use of minimally invasive surgical procedures. These include Epidural Steroid Injections (ESI), Facet Injections, Nerve Blocks, and Implantable Neurostimulation Devices. These methods may involve the cutting and/or removal of nerves and muscles.

Treatment Options

The board-certified partner physicians at Migraine Treatment Centers of America are specialists in interventional pain management. Depending on your medical history and past treatments, your physician may recommend:

  • Intramuscular narcotic injections to control pain. Demerol or Toradol are often used for this purpose.
  • Nerve block injections to selectively paralyze nerves that transmit pain. Botox, Depromedrome and Sarapin are frequently used as nerve blocks.

If intramuscular injections and nerve blocks fail to provide relief, your physician may recommend a medical procedure such as:

  • Epidural steroid injections
  • Facet injections
  • Surgical removal of nerves
  • An implantable neurostimulation device

Within the last 15 years, implantable neurostimulation devices have proven to be very effective in treating chronic pain — including chronic migraine headaches. While certain risks are involved with any surgical procedure, the risks associated with an implantable neurostimulation device are relatively small. And they are especially small with the minimally invasive Omega™ migraine procedure offered by Migraine Treatment Centers of America.

Unlike other surgical options that involve cutting and/or removing nerves and muscles, the Omega procedure does not alter the patient’s anatomy. During the procedure, wires are implanted under the skin in the patient’s forehead. Although they can’t be seen, these wires provide gentle neurostimulation to the occipital and supraorbital nerves that can cause migraine pain. The wires are connected to a small battery implanted under the skin near the upper buttocks. The battery is designed to be charged wirelessly, last up to 10 years and not limit physical activity.

Patients first undergo a four-day trial to confirm that they respond to neurostimulation before the device is permanently implanted. More than 80 percent of trials successfully reduce pain in qualified patients. The procedure has been featured in Cephalalgia, the journal of the International Headache Society.

Study 1 – Successful Treatment of Charcot-Marie-Tooth Chronic Pain with Spinal Cord Stimulation: A Case Study

This case study involved a 37-year-old man diagnosed with Charcot-Marie-Tooth (CMT) disease with pain of more than 20 years. He was implanted with an SCS device and patient pain and quality of life was assessed one and six months later. Results showed that SCS was effective in decreasing pain, improving quality of life and reducing medication consumption at both one and six months post-implant. In addition, the patient was satisfied with SCS treatment.

Study 2 – Ultrasound Imaging and Occipital Nerve Stimulation

This case study follows six patients diagnosed with refractory chronic daily headaches and occipital neuralgia who did not find relief through extensive medical management. The patients underwent successful percutaneous trials of bilateral octopolar occipital nerve stimulation under ultrasound guidance, followed by permanent surgical implantation. In this case series, ultrasonography provided real-time, safe, and reliable placement of ONS electrodes, providing an attractive emerging technology in the field of Neuromodulation.
